Diminished abilities or stage of disease that would result in a transfer or discharge:
When the resident becomes a danger to him/herself or others. Behaviors are common as this disease progresses. Our staff are taught redirection techniques. If after numerous attempts at redirection, the behavior is not lessened , staff must then document these attempts prior to giving a behavioral medication such as Ativan, noting within an hour if the medication was helpful in reducing that behavior. Medically, if complex nursing interventions are required 24/7, this would also result in discharge.
