Highlights
AgeSong at Lakeside Park is a secure assisted living community located near beautiful Lake Merritt, and only blocks from downtown Oakland. AgeSong's care is comprehensive and tailored through individualized service plans. These plans involve the residents' families, caregivers, social workers, physicians, therapists and others involved in a resident's life. All of these people contribute to a continuum of care ready to address changes and increased needs. Each individualized service plan utilizes holistic and specialized programs that support the different expressions of aging.

Staff certifications and training:
AgeSong believes in providing formal and informal training for all staff to understand and embrace our approach to care. Our method emphasizes the need to meet people where they are, approaching challenges openly and creating solutions. Our more formal trainings are developed through Pacific Institute, collaborating educational centers, hospice providers, and our own trainers. The trainings are supported by staff and residents working together to find the best approaches to care based on individual needs. These formal trainings are on a regular basis and Lakeside Park staff is required to attend.

Diminished abilities or stage of disease that would result in a transfer or discharge:
Lakeside Park is not a medical facility, so if the resident has medical needs that cannot be attended to by our staff, then they could be discharged. If a resident's behavior is harmful to other residents or harmful to their own self -- to the point that our trained staff cannot manage -- the resident may be discharged.
Special care programs offered for Alzheimer's and Dementia residents:
The Forgetfulness (Dementia) Program takes an interdisciplinary approach to caring for people with different types of dementia. We prefer to call our dementia program Forgetfulness Care, as it de-stigmatizes the symptoms of Alzheimer's and other forms of dementia. Pacific Institute mental health interns work in concert with AgeSong caregivers at Lakeside Park to "see" and relate to the person rather than just to treat symptoms like forgetfulness and confusion.
